Information We Collect
When you submit our intake form, we collect the following personal information:
- Full name, phone number, and email address
- Medication use history and diagnosis details
- Details about your medical condition and treatment
- Legal status (whether you have an attorney)
- Description of injuries and referral source
- Technical data: IP address, user agent, and submission timestamp
Claims Involving a Child
Some of the claims we review involve harm that began when the injured person was under 18. For those claims, the very first thing our form asks is an age range for how old that person is today, and the answer decides what we ask next.
If the injured person is still under 18, we next ask whether you are their parent or legal guardian. If you are not, the form ends there and nothing is saved. If you are, the contact details we collect and store are yours: your name, phone number, email address, and state. About the child we collect only two things, both as age ranges rather than exact ages: how old they are today, and how old they were when the harm began. We do not collect or store a child's name, date of birth, school, social media handles, usernames, or any other detail that would identify them, and our intake system refuses a submission to a minor-claim form that tries to send one. If a claim moves forward, the trial firm collects what it needs directly from you.
If the injured person is 18 or older today, they complete the form for themselves even when the harm began in childhood. There is no guardian involved, we do not ask for one, and the contact details we store are their own — the same as for any other claim on this site. In that case we store an age range for their age today and their answer to whether the harm began before they turned 18.
A parent or legal guardian may ask us to delete everything we hold about their family's inquiry at any time by emailing [email protected] or calling (424) 281-0286. We will confirm once it is done.
How We Use Your Information
We use the information you provide to:
- Evaluate your potential injury legal claims
- Contact you to discuss your case
- Refer you to qualified trial attorneys, with your consent
- Improve our services and website functionality
How We Store Your Information
Your data is stored securely using Cloudflare Workers KV, a globally distributed key-value storage service with encryption at rest. Intake submissions are retained for 90 days, after which they are automatically deleted. We do not sell your personal information.
Information Sharing
We may share your information with partner trial law firms for the purpose of evaluating and pursuing your legal claims. We will only share your information with a specific firm after informing you and obtaining your consent. We do not sell or rent your personal information to third parties for marketing purposes.
California Consumer Privacy Act (CCPA) Rights
If you are a California resident, you have the following rights under the CCPA:
- Right to Know: You may request disclosure of the categories and specific pieces of personal information we have collected about you.
- Right to Delete: You may request that we delete the personal information we have collected about you, subject to certain exceptions.
- Right to Opt Out: We do not sell personal information. If this changes, we will provide an opt-out mechanism.
- Right to Non-Discrimination: We will not discriminate against you for exercising your CCPA rights.
